Ordway Center uses a system of background verification checks to reduce the risk of making an inappropriate hire or being unaware of important job-relevant information for those hired in staff or volunteer positions. Ordway Center may request a background report which may include criminal conviction records; public court records; and when applicable, Department of Motor Vehicle records; or credit reports. Please be aware that you may be required to complete this background consent process during our hiring process and that your acceptance as a volunteer may be contingent on successful completion of a background check. I certify, to the best of my knowledge, that all information given by me/applicant in this application and in any other forms I/applicant complete during the application process is true and correct without consequential omissions of any kind whatsoever. I understand that false or misleading statements made by me/applicant or consequential omissions of any kind in the application process are sufficient cause for my/applicant not being accepted as a volunteer or for my/applicant being dismissed if I/applicant am already a volunteer no matter when discovered. I agree that Ordway Center shall not be liable in any respect if my volunteer employment is terminated because of falsity of statements, answers, or omissions made by me in this application.
I authorize Ordway Center to investigate all of the information in this application. Any persons or organizations named are authorized to provide information regarding my/applicant’s employment, school, volunteer history, character, and qualification and work, and they are hereby released from all liability for providing such information.
I understand that there will be an interview prior to my/applicant’s being accepted as a volunteer and I/applicant will be expected to complete the required orientation for volunteers. I understand that youth volunteers must be at least 14 years of age to be accepted. I understand that any volunteer under the age of 18 must have their parent/guardian accompany them to Volunteer Orientation.
